Abstract

|
|
Data on 374 patients operated at the department of surgery, Busan Gospel Hospital, with stomach cancer during the 3-year period from 1976 to 1978 was analyzed and the results were as follows.
1) Male to female ratio was 2: 1, and the highest incidence of age range was 6th decade. 2) In analysis of age range and survival, 5th decade had the highest 5year survival rate. 33.8%.
3) Curability was high in symptom duration of less than 3 months and over one year and the patients who had symptom duration of less than one month had the highest 5 Y.S.R. 40.5%.
4) The most frequent clinical symptom was epigastric pain (70.8%) and clinical sign was epigastric tenderness(38. 0%).
5) In analysis of tumor location and survival, 5 Y.S.R. of body cancer was 34.6%, 5 Y.S.R.
of-antrum cancer was 31.3%, 5 Y.S.R. of cardia and fundus was 18.2%.
6) Five year survival rate in subtotal gastrectomy group was 35.3%, in total gastrectomy
20%, in extended total gastrectomy 10%, and in proximal gastrectomy there was no survivor. 7) Cancer staging was used according to TNM classification by the manual of AJCC 1983.
Stage I occupied 5.2% of curable operation group with 5 Y.S.R 100%, stage II occupied 21.3% with 5 Y.S.R. 61.9% and stage IQ occupied 73.5% with 5 Y.S.R. 17.4%.
8) In curable operation group, 5 Y.S.R was 30.2%, 4 Y.S.R was 32.2%, 3 Y.S.R was 39.2%,
2 Y.S.R was 55.3% and I Y.S.R was 79.4%.
9) Resectability for cure was 61. 5%(230/374) and follow up rate in curative operation was 86.5%(199/230).
10) In 144 palliative group, 83 cases were resected and 61 cases were non-resected. In resected group, mean survival month was 9.8 months, 1/2 Y.S.R was 65.0%-and 1 Y.S.R was 30.0%.
In non-resected group, mean survival month was 5.8 months, 1/2 Y.S.R. was 34.2%, and 1 Y.S.R. was 17.4%.
11) Overall operative mortality rate was 2.4%(9/374) and curative operation motality rate was 0.8%(2/230).
12) Post operative complications occurred in11.7%(44/374).
13) The most frequent site of distant metastasis was liver(65 cases).
